Recovery Resources Care Manager - Full Time
A special opportunity for the nurse or social worker who is adept at advocating for patients who need access to resources while confronting mental illness, addiction, and abuse. This position also requires data management experience to develop an internal opioid registry to support Tri-County clinicians. This full-time position will rotate among our five health centers.
• Substance abuse experience a plus
• Masters in Social Work (MSW)
• RN, preferred BSN
Patient Service Advocate - Full Time
Want to play an important role at one of our growing health centers? This full-time opportunity puts you front and center! Yes, you’ll be greeting patients and doing so much more. You’re their champion starting with gathering their health information and later connecting them to resources they may need to be healthy. Insurance and prescription questions? You help connect them to the right person or organization. Our patients will come to rely on you.
Knowledge of medical terminology is a plus but we’re willing to train.
LPN Needed in Wayland- Full Time
A special opportunity for a family medicine nurse at our growing Wayland Health Center. Here you’ll provide outpatient care for families from pre-natal through adulthood. Certified Medical Assistant - Full Time
